【Famous Column】Dr. Zeng Yuwen / Suspect urinary incontinence? Simple 4-step self-test!

“What should the doctor do! I often want to run to the toilet in the middle of the night, at least 4 or 5 times back and forth! Also, it’s even scarier when I sneeze or cough during the day. The smell of urine is annoying…”

The 50-year-old mother Huang, who has given birth to two children, whispered about her troubles in life with an embarrassed expression on her face in the clinic.

Why do you leak urine? Take everyone to understand step by step!

Why does the bladder hold? Severely get “social cancer”?

According to the National Health Administration, nearly 3 million people in Taiwan suffer from urinary leakage. Among women over 40, 19.2% cannot control their urination in the past year, and 79.2% have leakage when coughing, laughing, sneezing or exerting force on their abdomen. Urine. Faced with such a difficult problem, only 17.7% of women seek treatment.

Many mothers have responded to me, why do babies leak urine after giving birth?

It is because the mother’s vaginal opening is enlarged to 3.3 times the width when the baby’s head passes through the birth canal during natural delivery, and 36% of mothers are torn due to strained pelvic floor muscles.

As long as the stomach exerts force, The pressure in the abdominal cavity increases, and the bladder receives pressure stimulation, which causes the detrusor muscle of the bladder to continuously contract, and the urethral sphincter cannot hold, and there will be a problem of urine leakage.

In addition to pregnancy and natural birth as common causes of urinary leakage, there are also factors such as weight gain, pelvic floor muscle injury caused by moving heavy objects, bladder damage caused by inertial holding in urine, and hormonal changes in menopausal women that cause mucous membranes to age and shrink.

Urine leakage, ranging from wet underpants to wet bottoms, or affecting life in severe cases, rejection of sexual behaviors, inability to go out, giving up exercising, and even fear of smelling urine, these are symptoms of “urinary incontinence”, and I would not dare to do so for a long time. Hang out with friends, low tide haze causes depression.

Know the types of urinary incontinence at a time

The most common types of urinary incontinence: stress incontinence, urge incontinence, mixed incontinence.

1. Stress urinary incontinence:
The main reason is that the pelvic floor muscles are weak to support, and the urethral sphincter cannot be completely closed due to the relaxation of the urethral sphincter, and the increased abdominal pressure will cause involuntary urine leakage.

It can be divided into 4 grades according to the severity:

Level 1 (mild): Urine leakage when coughing, lifting heavy objects, or jumping.
Level 2 (moderate): Urine leakage when sneezing, laughing, running, brisk walking, climbing stairs, mopping the floor.
Level 3 (severe): Leaks when walking, doing housework (eg sweeping the floor, washing dishes), changing posture (eg standing to squatting, squatting to sitting).
Grade 4 (very severe): Urine leakage when turning over in bed during sleep.

2. Urge incontinence: The
muscle layer of the bladder contracts involuntarily. Generally, the bladder can hold 200cc of urine. People with urge incontinence have a bladder of about 50cc. There is a crisis of urinary leakage”, which is common in people who are urgent to urinate, frequent urination, nocturia, and who often cannot hold back the urge to urinate and leak urine.

3. Mixed incontinence: A
combination of both stress and urge incontinence.

Self-test urine leakage at home

Here I will teach you how to use the “cotton pad weight” at home to detect the severity of your urine leakage! Come along and try it!

Before the measurement, you need to drink more than 500cc of water, and hold it for 30 minutes without going to the toilet.
Take out a clean, unopened sanitary napkin and weigh it with a small scale. Remember not to throw away the outer packaging, it will be used after the measurement.

After drinking water for 30 minutes, put on a cotton pad and start a 30-minute exercise. Here you can choose to climb stairs, jump rope, and run in place.
After exercising, weigh the urine-saturated cotton pad with the outer wrapping roll. Remember to deduct the original weight from the result of the scale!
According to the measurement results:

Less than 1g has no urinary incontinence problem
1-10g is mild urinary incontinence
10-50g is moderate urinary incontinence
More than 50g is severe urinary incontinence
The above measurements can be used to preliminarily test the degree of urine leakage, but it is recommended that you go to the gynecology department as soon as possible Or urology for evaluation and treatment by a professional doctor.
